School Nurse/Health Assistant Statement
School District Health Services would like to clarify and change a long standing misconception. The school health services program includes a staff of Registered Nurses and Health Assistants. School health offices are staffed by a Health Assistant, not a “nurse”. Health Assistants are trained in CPR, First Aid and medication administration. There are eleven Registered Nurses in District 51. Their offices are located in the Hawthorne Building and they do not have regular office hours in the school buildings. The Registered Nurse for Pomona Elementary School is Sheila Polhamus. Her phone number is 254-5404. The Health Assistant is Lorna Brady. The Health Assistant is in the Health Office at Pomona from 8:00 a.m. to 3:30 p.m. Monday through Friday and her phone number is 254-4990.
Prescription medications will NOT be given at school unless the following procedures are followed:
- Authorization for medication to be given at school form MUST be completed and signed by BOTH the PHYSICIAN and PARENT/ GUARDIAN
- Medications must be furnished in the original pharmacy-labeled container.
Non-Prescription medication will NOT be given at school unless the following procedures are followed:
- Authorization for Medications to be given at school
form MUST be completed and signed by PARENT/GUARDIAN.
- Medications MUST be furnished in the original container.
Authorization forms are available in the school office or at your physician’s office. To ensure the safety of your child, these procedures MUST be followed.
