School Nurse/Health Assistant Statement

School District Health Services would like to clarify and change a long standing misconception. The school health services program includes a staff of Registered Nurses and Health Assistants. School health offices are staffed by a Health Assistant, not a "nurse". Health Assistants are trained in CPR, First Aid and medication administration. There are 20 Registered Nurses in District 51. Their offices are located in the Hawthorne Building and they do not have regular office hours in the school buildings. The Registered Nurse for Pomona Elementary School is Camie Clear.  

The Health Assistant is in the Health Office at Pomona from 8:30 a.m. to 4:15 p.m. Monday through Thursday and 8:15 a.m. - 2:15 p.m .on Fridays.  You can reach both the Registered Nurse and Health Assistant through our main number (254-4990).

MEDICATIONS
Prescription medications will NOT be given at school unless the following procedures are followed:

  • Authorization for medication to be given at school form MUST be completed and signed by BOTH the PHYSICIAN and PARENT/ GUARDIAN
  • Medications must be furnished in the original pharmacy-labeled container.

Non-Prescription medication will NOT be given at school unless the following procedures are followed:

  • Authorization for Medications to be given at school form MUST be completed
  • Medications MUST be furnished in the original container.

Authorization forms are available in the school office, on MCVSD51 website through the parent and Nurses Services link, or at your physician's office. To ensure the safety of your child, these procedures MUST be followed.

 

 

 

A Note About Vaccinations

COLORADO LAW STATES:
IF YOUR CHILD'S VACCINATIONS ARE NOT UP-TO-DATE
HE/SHE CAN BE SUSPENDED FROM SCHOOL

  • If an immunization is against your religious beliefs, you must sign a religious exemption.
  • if your child cannot receive an immunization for medical reasons, a physician must sign a medical exemption.
  • If you have personal beliefs opposing immunizations, you must sign a personal exemption.
  • New Colorado Immunization Law

    As of July 1, 2016 in order to have a personal or religious exemption for your child's immunizations, you must sign a new form annually. ; Exemption forms can be obtained from the health office or online at:https://www.colorado.gov/pacific/cdphe/vaccine-exemptions ; Or from the school Health Assistant.

 

Did you know that your 5th grader will be required to have a Tdap vaccination to enter 6th grade?

Proof of Tdap vaccination is required by the first day of 6th grade. ; Please bring a copy of your child's immunization record showing a current Tdap vaccination to 6th grade registration. ; Your child can be prevented from starting school if there is no proof of a Tdap vaccination.

  • Your child's primary care physician
  • The Mesa County Health Department, 248-6900

Colorado state law requires Tdap vaccination for all 6th through 12th graders. This is an additional requirement, even if your child was up-to-date with all vaccinations when they entered kindergarten. ;
Tdap is a Tetanus, Diphtheria, and Pertussis booster. ; It provides the added protection of Pertussis (Whooping Cough) to the standard Tetanus booster.
